IHSA SPORTS PHYSICALS ARE REQUIRED TO BE UPLOADED FOR ALL ATHLETES
Club/Activities participants DO NOT NEED SPORTS PHYSICALS!
All students who want to participate in any tryouts, practices, and sports
MUST have the following 3 items completed in order to participate, NO exceptions:
1. Students MUST be registered for the sport they wish to participate in through the RSchool system.
2. Students MUST have a valid sports physical examination within the last 12 months uploaded into the RSchool system.
3. Athletic Fee Payment MUST be made through the RevTrak system.
If the 3 items are not done, the student will not be allowed to participate in tryouts, practices, and sports until all items are completed.

The IHSA Sports Physical Forms needed to participate can be Download by Clicking HERE
Students must have all of the above 4 pages of IHSA physical forms completed and uploaded,
just 1 or 2 pages of the forms uploaded is not a complete physical and will not be acceptd.
(PLEASE DO NOT BRING FORMS INTO THE SCHOOL OR ATHLETIC OFFICE - THEY MUST BE UPLOADED!)
Please note No other forms but the above IHSA ones will be accepted as a valid physical, immunization forms are not a valid sports physical.
