In consideration of this entry I hereby, for myself, heirs, executors, and administrators, waive any and all claims I may have for damages against School District 21, Holmes Middle School and staff, Frost Elementary School and staff, Twain Elementary School and staff, Whitman Elementary School and staff, all event sponsors, volunteers, and all individuals associated with this event, their representatives, successors, and assigns for any and all injuries suffered by me in connection with this event including pre and post event activities. I have been warned that I must be in good health to participate in this event, and I attest and verify that I am physically fit and have trained for this event. I hereby grant permission to School District 21, Holmes Middle School and staff, Frost Elementary School and staff, Twain Elementary School and staff, Whitman Elementary School and staff and their authorized agents to use my name and photographs, videotapes, or any other recording of my participation in this event for any purpose with no monetary payment to me. *
