Please complete the following Alternative Income Survey to help your school maximize its funding and to ensure you and your students receive the maximum benefits possible.  By providing your household size and income range, you are helping the District to qualify for extra funding that is required to be spent in support of our students.  Families and students may also benefit with discounted school and other fees, reduced utility costs, and other benefits. Some families may qualify for the assistance program known as P-EBT.

The information that you provide is confidential and compiled for reporting purposes to the State only.  It is not shared with outside groups or accessible to anyone other than those who process the data and send it to the State.

We are asking every family in DUSD to complete this form. Be sure to add the names of all of the students in your household at the end of the survey.
