For the safety of the students, School of Science and Technology- Champions implements the following procedures for students utilizing day care transportation. Parents are expected to cooperate with school authorities on these procedures to keep Champions as a safe, secure environment. If your child is to be transported to and/or from school to another location, please complete this form and return it to the front desk BEFORE the first day of school. Requests for day care transportation must be made annually.

Administration may approve temporary changes:
• for emergencies such as illness in the family, parent out of town, etc.
• Telephone requests will not be taken for permanent changes.
• Requests must be submitted on a Day Care Transportation Form and submitted to the front office.
• Notes signed by parents requesting permanent transportation changes will not be accepted.

While this may cause parents temporary inconvenience, it is critical we know at all times which children are approved for day care transportation.
