Optional Vaccination Status Reporting

Dear Parents/Guardians of D91 Students,
Last month, the COVID vaccination was approved for use with children five through eleven years of age. Now that most students are eligible to receive the vaccine, the district is providing parents the option to provide proof of student vaccination for COVID-19. This information will be used exclusively for contact tracing and to determine if a student needs to be excluded from school. Under current guidance, students that are fully vaccinated (two weeks from the second dose) do not need to be excluded from school when in close contact with a suspected or confirmed case unless the student is also exhibiting symptoms. Without a record on file, students will be assumed to be unvaccinated.
Families that would like to provide vaccine information are asked to fully complete the attached electronic form AND upload a picture/pdf of the student's vaccination card. The health office will maintain the student's name as well as vaccine administration details on file in order to respond appropriately to all current and future IDPH/ISBE protocols.
