Click on "Reserve A Spot" to the right of the screen. Follow the prompts to complete registration.
Please input the parent or guardian's name under "Billing information." The student's information will be added under "Registration." Once that is complete, you will enter the payment information and click "Place Order."
We strongly recommend that you bring a digital or printed copy of your sign-up and payment confirmation.
Appointments cannot be refunded or changed for any reason so please be sure to select the appropriate date and time. This process for completing annual sports physicals has been carefully considered and will be strictly followed.
Each athlete is asked to follow the below process:
- Arrive 15 minutes prior to the confirmed scheduled time
- Upon arrival, please report to the Check-In station that will be clearly identified.
- Upon check-in, the athlete's paperwork will be reviewed for completion and appropriate parent signatures. Once the paperwork is confirmed complete, the athlete will get in line for the medical physical assessment process.
You will be emailed a form titled "History and Evaluation" - Please print and fill out before the student's arrival. Please use only the Athlete's name on the form. This form must be signed by a parent or guardian.
